DEPARTMENTS - ID Card Dept
Since 1992 onwards CCBA has been entrusted by Cochin Port Trust authorities to co-ordinate with the I.D. card section of the CPT & the Chief Security office of CISF to issue identity cards to all port users. CCBA is still doing a commendable job in this regard. CCBA also assists the members in issuing the Customs I.D. cards.
1. CPT Permanent Wharf Entry cum Transaction Pass
This is applicable in the case of CHA organisations only.Inorder to apply for permanent Wharf Entry cum Transaction Pass at Cochin Port, A company/firm has to submit the following details.
a. Duly filled application form as per Form -A which is available at CCBA office in     triplicate.
b. 4 Stamp Size colour photos of the applicant
c. A cheque/ cash of Rs.150 per applicant
d. A covering letter addressed to the Traffic Manager with a copy marked to     ChiefSecurity officer, Cochin Port Trust
e. Copy of the CHA license
A maximum of 15 are only issued in the name of a company/firm at a time
2. CPT Wharf Entry Pass
This is applicable in the case of other organisations such as Steamer Agents., Stevedores, Ship hannelers,Exporters etc. Inorder to apply for permanent Wharf Entry Pass at Cochin Port, A company/firm has to submit the following details.

a. Duly filled application form as per Form -B as available at CCHAA office in     triplicate.
b. 4 Stamp Size colour photos of the applicant
c. A cheque/ cash of Rs.150 per applicant
d. A covering letter addressed to the Chief Security officer, Cochin Port Trust
e. Copy of the appropriate license issued by the concerned authority.

The applicants need to submit the details to CCBA office only.CCBA office scrutanise the same,submit the same to the concerned sections at the CPT and finally the I.D cards are signed by the Traffic Manager/Chief Security officer and the same issued to the parties from CPT. CCHAA office co-ordinates all these processes
3. TEMPORARY FORMS FOR ENTRY INTO THE WHARF
The forms are available at the CCHAA office.
4.Customs I.D. Cards
In cases where CHA organisations intending to recruit a new staff the application to be submitted to I&B Section of the Custom House will have to be routed through CCHAA office and the same has to be signed by either by the President/Hon- Secretary of the CCBA. As regards the other permanent I.D cards the forms are available at CCBA office.
a. Pink Card for temporary CHA staff
b. Yellow Card for permanent CHA staff
c. White Card for CHA license holders
d. Blue Card for Steamer Agents
Following requisite details have to be submitted to CCBA for renewing Old Card
1. Duly filled application form 3 copies
2. Covering letter address to Asst. Commissioner I & B
3. 3 Photos
4. Copy of the old pass
Following requisite details have to be submitted to CCBA for Fresh card
1. Bio-data form attested by President/Hon. Secretary (for CHAs) only
2. Conduct Certificate
3. Appointment letter
4. Duly filled application from 3 copies
5. Covering letter address to Asst. Commissioner I & B
6. 3 Photos
The applicants need to submit the details to CCBA office only. CCBA office scrutanise the same, submit the same to the I & B Dept. of Custom House
